Why does Outro still have my record if I’m no longer a client?

Because Outro provided care to you, we may be required to retain your record after your care ends. Medical-record retention requirements can come from state laws, professional obligations, billing rules, legal requirements, and compliance obligations.

Does that mean Outro staff can look at my record whenever they want?

No. Retaining a record is not the same as open access. Staff should only access your record when they have a legitimate reason related to their role, and access should be limited to what is needed for that task.

What are legitimate reasons staff might access my record?

Examples include responding to your records request, transferring records when permitted or authorized, resolving billing or payment questions, reviewing care previously provided, handling a safety or continuity-of-care concern, conducting compliance or quality review, investigating a privacy/security issue, or meeting legal or regulatory obligations.

Would Outro ever share my record without my permission?

In limited situations, yes. HIPAA allows or requires health care providers to share protected health information without written authorization in certain cases, such as for treatment, payment, health care operations, legal or regulatory requirements, safety concerns, or with vendors who support our services under appropriate privacy and security obligations.

That does not mean your record can be shared casually or with anyone who asks. In many situations, such as sharing records with an employer, family member, school, or unrelated third party, your written authorization would typically be required unless another legal basis applies.

What protections apply?

Access and sharing should be limited to legitimate purposes and, where applicable, to the minimum necessary information. Outro uses privacy and security practices intended to protect client records and prevent inappropriate access or disclosure.
